In this demonstration, you learn the preferred workflow for completing medication reconciliation on transfer.

Transfer MR should be performed at venue changes within the same encounter, including after procedures off the floor appropriate for a “time out”.

Reconciling the medications on transfer will display the following orders:

1. •All active and suspended historical and prescription orders across all patient encounters.
   

2. •All active inpatient orders across the current patient encounter.
   

3. •All active and previously active medication orders from the past 24 hours.
   

From the Orders Window,

1. Click the Reconciliation Button.

2. Select Transfer.

3. Make the appropriate selections from the Reconciliation Action.

In the situation where the preference is to only continue the current inpatient medications, a helpful workflow is to,

4. Choose the medication rows with an inpatient icon and select the appropriate radial button.

Then use Unreconciled Order(s) button (lower left screen) to quickly scroll to the next medication and radial button select.

Only when all medications have been addressed can the reconciliation be signed. 

5. Click Reconcile and Sign.
