In this demonstration you learn the preferred workflow for completing medication reconciliation on admission.
Medication Reconciliation is a button that allows the physician to efficiently reconcile a patient’s documented medication list and to quickly and accurately make the appropriate decision on each medication order.
Reconciling the medications on admission will display the following orders:
•All active, historical, and prescription orders across the current patient encounter.
•All active inpatient orders across the current patient encounter.
•All active and previously active medication order from the past 24 hours.
Notice how the logic of continuing or discontinuing with the appropriate radio button seems obvious.
Prior to the physician initiating medication reconciliation, the nursing staff records the home medication history from the patient. Once this has been completed, a green checkmark by the Status Meds History appears and the physician is now able to perform meds rec.
Now we are ready to complete the admission med reconciliation.
1. Click to open the Reconciliation button.
2. Select Admission.
Here, before selecting choices, a quick review and formulating a plan is quite helpful.
In order to complete the admission med req go line by line selecting to either Continue or Do Not Continue that line’s medication.
When you chose to Continue a medication it is converted to an inpatient order and is copied and listed under the Medications After Admission Reconciliation column.
Notice the (# Unreconciled Order(s)) button at the bottom of the window. You can click this button to take you to the next Unreconciled medication row.
An order can at anytime be added, even at the time of reconciliation.
3. Click Reconcile and Sign, only when all medications have been addressed.
