In this demonstration you learn the preferred workflow for completing medication reconciliation on discharge.
Discharge Medication Reconciliation is only one part of your patient’s discharge planning.
Discharge Medication Reconciliation can retrieve the following types of orders:
•Active prescriptions across all encounters.
•Prescriptions inactivated within the past 24 hours from across all encounters.
•Active documented medications from across all encounters.
•Documented medications inactivated within the past 24 hours from across all encounters.
•Active ambulatory pharmacy orders from across all encounters.
•Ambulatory pharmacy orders inactivated within the past 24 hours from all encounters.
•Active inpatient pharmacy orders.
To complete Discharge Medication Reconciliation:
From the Inpatient Summary,
1. Click the Medications header.
2. Click the Reconciliation button.
3. Select Discharge.
In this case the plan is to continue all the patient’s home medications, including the ones suspended in the hospital. Notice how the medications are divided between Home Medications that were suspended, Continued Home Medications, and Medications only given in the hospital.
First select the inpatient version of Zoloft…
Notice it is now writing a prescription for a medication that this patient already has. Oops mark Do Not Continue and Continue the home medication for Zoloft.
This patient will need a new medication prescription for Pantoprazole.
4. Click Add to place a New Rx.
5. The order window opens, Find Pantoprazole, select 40mg PO qDay #30. Click OK and Done.
Select Routing allows choosing prescription methods to include ePrescribing, printing the prescription to sign or Do Not Send with a variety of reason options. Further details about routing will be discussed during instructor classes and other supporting documentation.
6. Select Do Not Send: Given to Patient
7. Select Reconcile and Sign
